Register online at www.RayReid.com or complete the online registration application (or the brochure application if you received one in the mail). If registering by mail, send camper application and appropriate payment to:

Please note, payment (partial or full, depending on the program) and your parental consent form must be submitted within two weeks of your online registration. A payment needs to be enclosed for each camper you are enrolling and we ask that you please pay special attention to the deposit deadline. Also, please be sure to get your final payment and your player's medical forms in by June 1. We cannot guarantee spots after the deadline has passed. Finally, please be sure to fill out the application in its entirety.

CAMP PAYMENTS: Deposit & Balance
The Ray Reid Soccer School accepts both credit cards (MasterCard, Visa, American Express and Discover) and checks for camp payment.
Credit Cards*: If you choose to pay with a credit card, you are required to pay the entire camp fee at the time of registration. When paying with your credit card during the registration process, PLEASE make sure to only click once on the pay now option. If you are not sure if your payment has gone thru, please call the camp office at 860.674.1500 and it can be checked out for you. If you choose to pay by credit card, please be aware that "Gold Orluk & Partners" will appear on your billing statement in lieu of "Ray Reid Soccer School."
*Reminder: Those paying by credit card are required to pay the entire camp fee at the time of registration.
Checks*: Registration may also be made by check. Elite Academy registrants must make a deposit of half the cost of camp within two weeks of registration with the remaining balance due by June 1, 2010. CAMP REGISTRATION/CHECK-IN: SOCCER ACADEMY HELD AT THE UNIVERSITY OF CONNECTICUT

Registration for all Elite Academy campers (including Extended Day Campers) will take place in Gampel Pavilion on Saturday, July 17th (for Session I) and on Thursday, July 22th (for Session II) from 11:00 a.m. - 2:00 p.m. for each session.
At registration, you will check-in, receive your team assignment, room assignment and room key (a room key deposit of $100 is required at this time), and have the option of leaving money for the Camp Store. At check-in, you should bring any Medication Authorization Forms for the camp medical staff. At this time, you will also need to pay any outstanding balances that you have and submit any outstanding forms with the camp administrators. After registering at Gampel, players will move into their assigned dormitory room.
PLEASE NOTE: Lunch will not be served on the first day of camp - we encourage you to eat lunch before you arrive at registration or you can get lunch at the Student Union across the street from Gampel (options include: Blimpie's, Panda Express and Wendy's).

CAMP REGISTRATION/CHECK-IN: YOUNG STARS AND SPECIALTY CAMPS
For the Young Stars and Specialty Camps, we encourage campers to arrive a half hour early on the first day of each camp session to get registered for the week.

ELITE ACADEMY EXTENDED DAY INFORMATION
Campers attending the Elite Academy as an extended day camper must be dropped off at 7:30AM and picked up at 9PM at Gampel Pavilion and must be signed in and signed out by a parent each time. Extended Day campers will not receive breakfast but will receive lunch and dinner in the camp dining hall.
Extended Day Camper's experience a full day of soccer just like any other camper at the Elite Academy in Storrs including 3 training sessions a day. The first 2 training sessions are in the morning and afternoon and are focused on technical development, functional play, small-sided team tactics, attacking with numbers, transition, defending, and finishing. At night each camper will play full-sided games with the group they are assigned. They will be placed into a group upon arrival at camp that is based on the campers age and ability.

ACCOMMODATIONS FOR CAMPERS STAYING FOR BOTH SESSIONS
Campers may stay overnight in UConn housing between Session I and Session II for an additional cost of $80.00. This fee will cover their room for the night, meals and supervision from the coaching staff. To make these arrangements, please call the event office at (860) 674-1500.

Hotel Accommodations for players and parents are available at the Nathan Hale Inn, conveniently location on the University of Connecticut Campus. OTHER FORMS - LATE ARRIVAL, EARLY CHECK-OUT, ETC.
If your child will be arriving late to camp, you must fill out the Late Arrival Form and mail or fax (860.674.1704) it to the event office before the start of camp. If your player needs to leave earlier than the regular check-out time, you must fill out the necessary paperwork - Early Check-Out From - and hand it in when you check your player in to camp. Additionally, if your child has special camp scheduling needs (leaving camp mid-session for any reason and returning), please make sure that you call the Camp Office at 860.674.1500 to notify of this situation. For the safety of your player, if you have not filled out the early dismissal/permission to leave camp paperwork, your child will not be able to leave camp early.

CAMPERS WITH CARS
Campers planning to drive themselves to camp must notify camp staff before camp by calling 860.674.1500. If anyone else will be riding in that vehicle, both the driver's parent and passenger's parent must send a signed note to the event office giving their players permission to drive other campers/ride with other campers. Once at camp, keys must be handed in at registration and will be returned at check out. The camper's car will not be accessible through the duration of camp. Please understand that this policy is in place to protect your player and other campers.

A KEY DEPOSIT OF $100 IS REQUIRED AT CHECK-IN AND WILL BE RETURNED PROVIDED THE KEY IS TURNED IN AT CHECKOUT.
Please bring a separate check to registration for $100 made out to the "Ray Reid Soccer School." . Please do not mail the key deposit check in advance. This check will be returned to you when you return the key at check-out. The camper will not be able to receive a key for his/her room if the $100 dollar key deposit is not paid in full at registration.

PHYSICAL/MEDICAL RELEASE FORM - DUE BY JUNE 1st
Please print off the Ray Reid Soccer School Medical Release Form from the Forms page on this website. This information needs to be completed by your player's physician and mailed/faxed to the attention of the: Ray Reid Soccer School, c/o Gold, Orluk & Partners, P.O. Box 1177, Avon, CT 06001 as soon as possible following Registration (A physical form issued by the state of Connecticut within the last 2 years is acceptable - all campers must have a completed Physical Form to participate at one of our Ray Reid Soccer School Camps). We do not keep medical forms on file from year to year, therefore you will need to provide us with new forms each year your child attends camp.

MEDICAL STAFF
The Ray Reid Soccer School will have an athletic trainer working with each of its camps at all times. For more serious problems at the Elite Academy, the camp will use Windham Hospital, which is located approximately 15 minutes away from the University of Connecticut. The Ray Reid Soccer School, Inc. provides secondary accident/medical insurance.

CAMP BALL
ELITE ACADEMY: Each Elite Academy camper must have a soccer ball. You may bring your own ball or purchase one from us (use the check-off box on the online registration or on the camp brochure application to order one). Many campers choose to purchase an adidas soccer ball from us in advance, at the cost of $30, by clicking the box on the registration page. If you choose to purchase a ball at registration, the cost will be $35, if available.
YOUNG STARS CAMPS: All Young Star Campers will be provided with a complimentary Ray Reid Soccer School Ball as part of their registration fee and do not need to purchase a ball.

ADULT/COACH SUPERVISION AT THE ELITE ACADEMY
Camp staff members sleep in the dorms with the campers for the Elite Academy and the UConn police patrol the campus, including the dorms, 24 hours a day. Attendance roll is run regularly.

COACH TO CAMPER RATIO
The Ray Reid Soccer School works to provide a coach to camper ratio for all camps of one coach for each ten campers registered. It is for this reason that the cancellation and down payment policies are enforced..

PLAYER EVALUATIONS AT ELITE ACADEMY
All Elite Academy campers will receive a written Player Evaluation at the conclusion of the camp week. In addition to an evaluation of each player's technical and tactical level, we will offer a strength and conditioning assessment as well (it will be run by collegiate strength and conditioning coaches and will record key areas in speed, power, and agility).Players will leave the camp with hard numbers in key physical areas that can be relayed on to college coaches in the recruiting process and be provided a skill assessment that will help them on their road to becoming better players.

TEAM/TRAINING GROUP ASSIGNMENTS BY AGE AND SKILL LEVEL
Team and training group camp assignments are done by age and ability. The camp coaching staff will evaluate all players and place them with appropriate age and skill levels. Teams attending the Ray Reid Soccer School can train and play as a team at camp - arrangements for these training groups needs to be made in advance by calling the event office at 860.674.1500 to request that your group train as a team.

ROOMMATES AT THE ELITE ACADEMY
The Ray Reid Soccer School does its best to pair campers together as roommates in instances where both campers request each other. If your son does not request a roommate, we do our very best to find and assign a roommate that is the same age. Please note that the dorm rooms allow for only 2 campers per room (for groups of three, we do our best to put these players in adjacent rooms - please call the Event Office with these requests or to learn more).

MEALS AT THE ELITE ACADEMY
Each of the three meals offered during each day will be served in a UConn dining facility. All of the food is served buffet-style and is "all-you-can-eat" at each meal. There is a wide selection of meals, including a vegetarian option at each meal. They usually have a pasta bar at dinner, regular hot food service, a sandwich bar, a salad bar, lots of fruit and drinks, ice cream, etcÉ IMPORTANT/FOOD ALLERGIES: Please notify the camp administrators at 860.674.1500 or by email at info@RayReid.com if your son has any food allergies.

PHONES IN THE DORMS AT THE ELITE ACADEMY
There are no pay phones in the dorms. If campers bring a cell phone (at their own risk), please note that the cell phone may only be used in their dorm room. All cell phones must be turned off at "Lights Out" and cannot be brought out to the fields during practice/games. The Ray Reid Soccer School cannot be held responsible for lost or stolen cell phones/IPODS/computers.

MATCH SCHEDULE/CAN I WATCH MY SON PLAY AT THE ELITE ACADEMY
You can watch your son play at the Elite Academy during the Evening Soccer Matches which start at approximately at 5:30PM each night of camp. The Ray Reid Soccer School will do its best to post a Match Schedule on its website the week of Elite Academy Camp.

CAMP STORE AT THE ELITE ACADEMY
We have a Camp Store that is available to all of our Elite Academy campers. We DO NOT encourage campers to carry cash. You have the option of leaving money for your child in our Camp Bank. This will ensure that your child does not lose his money during camp. We ask that parents make a cash deposit into the Camp Store at registration. Your son then charges against that amount all week. At camp check-out, we refund the money that your son has not spent. At the Camp Store we sell pizza, snacks, drinks and other soccer items.

CANCELLATION/REFUND POLICY
2010 Elite Academy Cancellation/Refund Policy: If you need to cancel your player's registration for any reason prior to June 1, 2010, a cash refund - minus a $200.00 administrative fee - will be given for cancellations submitted in writing before June 1, 2010 OR we will issue you a camp credit for all fees paid, valid through 2011. All cancellations after June 1st and through July 3rd, 2010 are only eligible (if approved by camp staff) for a camp credit for a 2011 Ray Reid Soccer School Camp session. Refund requests submitted in writing following July 3rd, 2010 for a 2010 Elite Academy Camp will only be considered if accompanied by a physician's note indicating that your player is unable to participate for medical reasons - this refund will only be provided in the form of a camp credit for a 2011 Ray Reid Soccer School Camp.
2010 Young Stars/Specialty Camps Cancellation/Refund Policy: If you need to cancel your player's registration for any reason prior to June 1, 2010, a cash refund - minus a $50.00 administrative fee - will be given for cancellations submitted in writing before June 1, 2010 OR we will issue you a camp credit for all fees paid, valid through 2011. All cancellations after June 1st and received 7 days in advance of a Young Stars Camp date are only eligible (if approved by camp staff) for a camp credit for a 2011 Ray Reid Soccer School Camp session. Refund requests submitted in writing within 7 days of the start of a Young Stars Camp will only be considered if accompanied by a physician's note indicating that your player is unable to participate for medical reasons - this refund will only be provided in the form of a camp credit for a 2011 Ray Reid Soccer School Camp.
All requests for a refund/credit of any kind must be submitted in writing (Ray Reid Soccer School, c/o: Gold, Orluk & Partners, LLC, PO Box 1177, Avon, CT 06001), email (info@rayreid.com) or fax (ATTN: Ray Reid Soccer School- 860.674.1704) and will not be accepted over the telephone. All camp credits are only transferable to a family member.

EARLY DEPARTURE/NO SHOW POLICY FOR ALL CAMPS:
Any camper who leaves camp early because of an injury (physician's note required) will receive a prorated credit for 2011 for the time they missed at camp. Campers who voluntarily choose to leave early or are asked to leave for disciplinary reasons will not be refunded in any way. Campers who do not show up will not be eligible for a refund of any kind.

CAMP CHECK-OUT/CLOSING DAY AT THE ELITE ACADEMY
CLOSING CEREMONY: Friends and family are invited to attend a brief awards ceremony on the last day of camp that will begin at approximately 2PM outside of McMahon Hall on the UConn campus. Championship Games will take place prior to the ceremony.
CHECK-OUT: Official check-out for each session of the Elite Academy will take place on the last afternoon of camp at approximately 2:30PM outside of McMahon Hall. At check-out, you will move out of your dorm room and be required to return your room key in order to receive your deposit. You will receive any remaining Camp Store funds and any medications that were left with the camp medical staff.
EARLY DEPARTURES: Plans to take any camper home before the official camp check-out time must be cleared with the Camp Director ahead of time (Please see OTHER FORMS).
Please Note: If someone other than a camper's parent/guardian will be coming to pick up your child from camp, a permission note from that camper's parent/guardian must accompany the adult picking up the camper. If possible, please notify the camp administrators in advance by calling 860.674.1500. Please bring a signed note from that parent/guardian to check-in on the day of registration.

GROUP DISCOUNTS TRAIN AS A TEAM
In order to receive a group discount, you must send in all the players' applications together.
GROUP DISCOUNTS: Elite Academy I & II Groups: In order to receive group rates*, ALL REGISTRATIONS AND PAYMENTS MUST BE SENT IN TOGETHER. Campers may register ahead of time online and mail in check with team; however, if payment for group does not come in together, the group rate will not be honored.
- Groups of 10-14 campers receives a $10 discount per camper;
- Groups of 15+ campers will receive a $25 discount per camper.
If you send 10 or more campers as a team to one session and your coach would like to attend free, please contact the Event Office at 860.674.1500 to discuss details.
ALL TEAM MEMBERS MUST FILL OUT THE TEAM INFORMATION BOX ON THE CAMP APPLICATION.
*Special group rates cannot be combined with any other special offer discounts such as the Early Registration/Holiday Discount Offer.

TRAIN AS A TEAM
If you are interested in having your team train together during camp, please contact the event office at 860.674.1500 to discuss "Train As A Team" opportunities and to make necessary arrangements. We may have you speak directly with one of the Camp Directors so that we can best accommodate your group and achieve your team goals.

WHAT TO BRING
- Cleats that are already broken in.
- Indoor shoes for possible indoor sessions and casual evening wear. Sometimes because of bad weather sessions will be conducted indoors.
- A warm-up suit and/or rain gear
- Plenty of tee shirts and shorts
- Linens, blankets, pillow and towels.
- Sandals for the showers
- Bathroom items e.g. soap, shampoo, toothbrush, toothpaste deodorant
- A fan. None of the rooms are air-conditioned
- A bathing suit. Some camps have access to a swimming pool.
- An alarm clock. You don't want to be late for a session
- A calling card or change so you can call home.
- Bug-spray and sun-screen.
- Spending money for evening snacks, laundry or the camp store.
- Athletic tape and wraps if you need them. There will be a trainer on hand to assist you.
- A ball! You cannot play without one.
- A water bottle.

The camp does not provide transportation to or from any airport or bus terminal due to liability and the demands of the entire staff at registration and camp closing.

Campus Parking for Elite Academy
Parking should be available in UConnŐs South Parking Garage for a fee. South Parking Garage can be accessed via the lower entrance off of Stadium Road (take a right immediately after Gampel onto Stadium Road and then a left into the lot) or via the fifth floor entrance/exit off of Lot 8. To enter via the upper/Lot 8 entrance, you will take Hillside Road past Gampel Pavilion (on your right) and the UConn Co-Op (also on your right), then take the right immediately after the UConn Co-Op up the hill (located between the Museum of Natural History and the the UConn Co-op) to enter the South Parking Garage. Gampel Pavilion and McMahon Hall are within easy walking distance of the South Parking Garage.

Elite Academy Camp Playing Fields Directions
UConn #2 Fields: Out of McMahon driveway, take a left onto Hillside Road and follow until you reach the next stop sign in front of the Co-Op and take a left (Stadium Road). Follow it and the field will be after the 2nd street on your right. It is opposite the Soccer Stadium.
UConn AstroTurf: Out of McMahon driveway, take a left onto Hillside Road. Follow until you reach the Co-Op. At the next stop sign, take a left. Take the first right after Gampel Pavilion (before the old Football Stadium). Follow the road to the Turf Field next to the football locker rooms and behind the Field House, it will be on your left.
Post Office Fields: Go straight out of McMahon driveway and stay on Gilbert Road. Turn right at the stop sign (Mansfield Avenue) and bear right as you approach the traffic light. Turn right at the light onto Rte. 195. Go to your second light and take a left in the Post Office road. Fields are past the post office on the right side.
E.O. Smith High School: Go straight out of McMahon driveway and stay on Gilbert Road. Turn right at the stop sign (Mansfield Avenue) and bear right as you approach the traffic light. Turn right at the light onto Rte. 195. E.O. Smith High School will be on your right, the Soccer Field is located behind the school.
Mansfield Middle School: Go straight out of McMahon driveway and stay on Gilbert Road. Turn right at the stop sign (Mansfield Avenue) and bear right as you approach the traffic light. Turn right at the light onto Rte. 195. Turn right at the light onto 275. Turn left onto Maple Road. There is a small yellow divider at the start of Maple Road and the Mansfield Senior Center is on the right side of the road. Follow Maple Road. You will reach a stop sign where Maple Road turns into Spring Hill Road. Go straight at the stop sign and MMS will be on your left in approximately .1 miles.
Memorial Field: Out of McMahon driveway, take a left onto Hillside Road. Follow until you reach the Co-Op. At the stop sign, take a left onto Stadium Road. Memorial Field will be on your right.
